Paperwork Burden Reduction Act

To retrieve your 1095-B from your UHC account 

1. Sign in to your UHC account

2. Click Tax Documents under the My Coverage tab

3. Select the desired Form 1095-B

4. View, download, or print a copy of your Form 1095-B

To request a paper copy of your 1095-B 

1. Call customer service at 1-800-767-0700

2. Email a request to customer service

3. Mail a written request to:

   • Form 1095-B, P.O. Box 30979, Salt Lake City, UT, 84130-0979
