About the Role
Provide a broad range of infusion therapies in our patients’ homes
Initiates, develops and implements nursing plan of treatments.
Assesses patient needs and physical status at each visit through health data systems and patient review. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders.
Provides effective and safe teaching for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals.
Have excellent written and verbal customer service skills in order to interact with the patient, physician, pharmacist and other partners.
Communicates with Sisu’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines.
Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care.
Demonstrates compliance with agency operations, Sisu Healthcare Solutions policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards.
Maximizes work efficiency using computers and other automation technologies to validate plan of treatment orders, visit records and follows assigned visit schedule
Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Sisu Healthcare Solutions information including adherence to HIPAA regulations.
Actively participates in facility process for Performance Improvement Program and assists Supervisor or Nurse Manager with chart audit functions as applicable.
In this role, you will work a flexible weekly schedule visiting patients in various cities such as; Bullhead City and Tucson and surrounding areas which may require travel of a 1-hour radius outside the normal territory (travel time is paid and mileage program) Participate in the on-call rotation as indicated by staffing and business needs. Ideally, the Registered Nurse would live in one of these locations and be able to work per diem.
Requirements
Registered Nurse with current unrestricted license in state of Arizona and potentially Nevada.
Minimum two-year nursing experience; IV-related experience a plus
Have and maintain current CPR certification
Possess a valid and current driver’s license and auto insurance
Have experience with Microsoft Office, Excel, Outlook and Word to document and track patient care activity.
Participate in on call rotation as indicated by staffing and business needs.
Education
RN Diploma, B.S. degree in nursing
